Forms and Screenings

No, submitted forms are not stored for your retrieval. However, the information on the form is used to complete your wellness program. Health metrics, preventive examinations, and tobacco use status is tracked within your account on the wellness portal and mobile app.

You can find all the forms for your wellness program in the Wellness Locker on the Portal.

You can email your form(s) to forms@wellworksforyou.com, or upload them through Contant Us on the mobile app and/or wellness portal.

This may depend on the Biometric Screening partner. Some results are updated within 7-10 business days, while others may take longer to process. At minimum, please allow 7-10 business days for participation to be updated on the portal and mobile app.

Faxed forms do not result in a confirmation email until they have been processed by our team. You will need to email the form(s) to forms@wellworksforyou.com to receive a confirmation of receipt. Then, you will receive a second email once participation has been updated.
